Any doctor is able to follow your care while at Elmhurst
Extended Care Center, if they choose. Currently, we have
an extensive list of over 80 physicians who are credentialed
to follow patient care here. If your physician is not able to follow
your care you may choose one of our excellent staff
physicians while at Elmhurst Extended Care Center.

Medicare will pay for your stay at Elmhurst Extended Care
Center if your condition meets Medicare criteria. The
main criteria is that the resident have a skilled need
that requires the daily involvement of a licensed nurse
or therapist.
Another requirement of Medicare
is that your stay at Elmhurst Extended Care Center should
be preceded by at least a 3 day stay at a hospital or
within 30 days of discharge from a hospital if there
is still the need for the daily involvement of a licensed
nurse or therapist. The Medicare program is very complex,
and you can contact the admissions staff here with specific
questions.

Medicare will fully pay for all medically necessary services
for the first 20 days as long as the resident’s condition
continues to require the daily involvement of a licensed
nurse or therapist. When a resident’s condition still
meets the Medicare criteria past day 20, Medicare will
pay for all medically necessary services except for
the coinsurance amount. The coinsurance amount increases
yearly in January. To find out the most current coinsurance
rate please contact the admissions staff or e-mail Elmhurst
Extended Care Center at eecc@ameritech.net. Typically the coinsurance amount is covered
by supplemental insurance. If you do not have supplemental
insurance or your policy does not cover the cost, the
coinsurance is the resident’s responsibility.

The maximum benefit period Medicare will pay for is 100
days for any illness. For most residents their condition
improves so they no longer need Medicare skilled services
before they reach the end of their maximum benefit period.
In order to receive a full 100 days of skilled coverage
again, the resident must have a 60 day break between
illnesses.

5.
First and foremost, we ask that the
resident or resident representative tour Elmhurst Extended
Care Center. This makes adjusting much easier for all
parties involved. Arrangements can be made for our Social Service Director
to come out and evaluate a future resident. A person that is coming
to Elmhurst Extended Care Center must also have a physical
completed within 24 hours of being admitted and physician
orders of medications, etc.

Elmhurst Extended Care Center is not able to follow a
resident’s care when they return home, however we do
make recommendations or referrals to home health agencies
to make the resident transition back home smoother.

7.
Yes, Elmhurst Extended Care Center has a specific unit
dedicated to taking care of people with Alzheimer’s and
other dementias. The Alzheimer’s Unit is certified under
the Illinois Alzheimer’s Special Care Disclosure Act.

Yes, Elmhurst Extended Care Center will care for residents
who require respite care. A minimum stay of seven days
is required.

9.
Yes, Elmhurst Extended Care Center does accept patients
who are currently under hospice care. If a person is
receiving hospice care at home or in the hospital, hospice
is able to continue their plan of care while the resident
is at Elmhurst Extended Care Center. Also if a person's
condition changes to where they require hospice care
while at Elmhurst Extended Care Center, we will encourage
hospice involvement when family and the physician are
also in agreement.
