Elmhurst Extended Care Center also has an extensive list of over 80 area physicians who are currently credentialed to follow the care of residents here. Any physician is welcome to follow a patient’s care while at Elmhurst Extended Care Center after completing the credentialing process.

Care Plan Team
In an effort to continuously respond to the specific requirements of each resident, Elmhurst Extended Care Center offers a care plan meeting which is based solely on the individual’s needs. Care plan meetings are held shortly after admission and are followed up by quarterly update sessions throughout the year. Residents and their families are encouraged to participate in these sessions. Care plans offer peace of mind to the residents and their loved ones.

Who is the Care Plan Team?
The care plan team is an interdisciplinary team that is made up of the professional staff from all aspects of a resident’s care.

The care plan team is lead by the Care Plan Coordinator - the nurse manager who oversees each patient's care plan and coordinates the care plan meetings. The other members of the care plan team are the Charge Nurse from the specific resident’s unit, the Dietitian, the Social Service Director, the Activity Director, and the Restorative Nurse Manager. If the resident is participating in therapy, the Physical or Occupational Therapist is also part of the care plan team.

Care plan meetings give families an excellent opportunity to meet key personnel that are involved with their loved one's care.
