Can my doctor follow my care while at Elmhurst Extended Care Center?

Any doctor is able to follow your care while here, if they choose. Currently, we have an extensive list of over 80 physicians who are credentialed to follow patient care here. If your physician is not able to follow your care, you may choose one of our excellent staff physicians.

Does Medicare pay for my stay?

Medicare will pay for your stay at Elmhurst Extended Care Center if your condition meets Medicare criteria. The main criterion is that the resident has a skilled need requiring the daily involvement of a licensed nurse or therapist. If you are coming from the hospital, a social worker will contact us and make arrangements to send us medical records. You can always call our admissions staff directly with questions and to plan a visit.

If I am coming from home, what is needed?

First, we ask that the resident or resident representative visit Elmhurst Extended Care Center. No appointment is needed and tours are a wonderful way for families to get to know us. Arrangements can be made for our Social Service Director to come out and evaluate a future resident as well. You should have a physical completed within 24 hours of arriving and physician orders for medications, etc. Our Admissions Team will let you know what to bring with you and what we will provide.

Will Elmhurst Extended Care Center provide services in the home after discharge?

We are not able to follow a resident’s care when they return home. However, we do make recommendations or referrals to home healthcare agencies that will help make your transition back home smoother. Our Social Services Team will be happy to help with your questions.

What about Respite Care?

We do offer short-term Respite Care for residents with a minimum stay of seven days.

Will Elmhurst Extended Care Center accept a patient who is under hospice care?

Yes, Elmhurst Extended Care Center does accept patients who are currently under hospice care. If someone is receiving hospice care at home or in the hospital, hospice is able to continue their plan of care while here. Also, if someone’s condition changes to where they require hospice care while here, we will encourage hospice involvement in agreement with family and physician.

What are the private pay rates for long-term care and Alzheimer’s care not covered by Medicare?

Our current private pay rates are available by emailing us at info@eeccmail.com. You will find that our daily rates are much less than other healthcare centers, and yet we offer the finest in nursing care. Private pay is a daily rate that is charged monthly. The daily room rates include licensed and certified staff to provide 24-hour nursing care, social services, seven-day a week activities program, accessibility to recreational areas and flat screen TVs, library, movies, dining service that includes three meals a day, snacks and specialty diets, housekeeping and linen service.
